Hi Gary,
The bushing that is inside the large center arbor hole of the blade?
If that is what it is, then the nut and backing were loose or when tight
left a hollow area that allowed the bushing to pop out. Keeping a washer
snugged up tight against the blade bushing is all you need to do.
